For many, coffee is a morning essential, providing the energy boost needed to start the day. However, a doctor has issued a cautionary warning about the potential downsides of drinking coffee first thing in the morning—especially on an empty stomach.
Speaking on behalf of Jude, GP Dr. Masarat Jilani explained that while your morning coffee may not cause immediate issues, it can lead to long-term gut sensitivity, particularly as we age.
Why Coffee Before Breakfast Can Be Harmful
Dr. Jilani warns that drinking coffee on an empty stomach can “irritate the gut and ramp up acid production,” increasing the risk of acid reflux.
According to WebMD, acid reflux occurs when stomach acid flows back into the esophagus, leading to discomfort and symptoms like heartburn. This happens when the lower esophageal sphincter (LES), which typically closes after food passes through, doesn’t function properly.

How to Reduce the Risk of Acid Reflux
If you can’t imagine starting your day without coffee, Dr. Jilani suggests waiting at least an hour after waking before taking your first sip. Ideally, you should eat something beforehand to help protect your stomach lining.
“Waiting about an hour and a half allows your natural cortisol levels to rise on their own while giving you a chance to eat something first, reducing the risk of stomach irritation.”
Spacing out your caffeine intake can also help maintain more stable energy levels, preventing the sharp highs and crashes associated with drinking coffee too early.
Coffee and Bladder Sensitivity
For those with bladder issues, coffee can increase urinary frequency. Dr. Jilani recommends trying decaf or half-caffeinated options to see if they have a milder effect.
“Everyone’s caffeine tolerance is different. Being mindful of how your body reacts can help you manage unwanted side effects.”

Dr. Jilani also advises avoiding caffeine after noon, as it can disrupt gut health, bladder function, and overall energy balance.
